Treatment-Resistant Depression
Treatment-resistant depression means depression that has not improved enough after at least two adequate courses of antidepressant treatment.
Persistent low mood, loss of interest, poor sleep and energy, and difficulty functioning despite treatment.
See a psychiatrist for a full review — do not stop your medicine on your own. Get urgent help at once if there are thoughts of self-harm.
Overview
Most people with depression improve with the first medicine or talking therapy they try — but not everyone. When two or more properly tried treatments have not helped enough, clinicians call it treatment-resistant depression. The label describes the treatment history, not the person: it does not mean untreatable, and it is a sign to review the diagnosis, the dose and duration, anything else that may be contributing, and the range of further options with a psychiatrist.

Low mood or emptiness most days, despite treatment

Sleep problems — too little, broken, or too much
Low energy and slowed thinking or movement
Poor concentration and indecisiveness
Changes in appetite or weight
Feelings of worthlessness or guilt

Causes & risk factors
There is no single reason a depression resists treatment. Possibilities a psychiatrist reviews include: the dose or duration was not yet adequate; another condition is present alongside (such as thyroid problems, anxiety, trauma-related conditions or substance use); the original diagnosis needs revisiting (for example, bipolar depression responds differently); ongoing life stress; or the medicine not being taken as prescribed, sometimes because of side effects. Finding the reason is the first step to a plan that works.
How it is diagnosed
A psychiatrist reviews the full history: which treatments were tried, at what dose and for how long, what helped even partly, side effects, physical health, other conditions, and safety. Blood tests or other checks may be used to look for physical contributors such as thyroid problems. Only after this review is the 'treatment-resistant' label applied — and the review itself often reveals the next step.
Treatment options
Options a psychiatrist may consider, in sequence, include: checking the dose and giving a treatment enough time; switching to a different antidepressant; combining medicines (for example adding a second medicine used to strengthen the effect); adding structured talking therapy such as CBT; treating anything found alongside; and, for some people, specialist procedures available at larger centres. Lifestyle foundations — sleep, activity, routine, and support — remain part of every plan. Changes are made one step at a time so it is clear what is helping.
Living with it — self-care
Living with depression that has not yet lifted is exhausting, and it is common to blame yourself — please do not. Keep appointments even when you feel nothing is working; tell your clinician honestly what has and has not changed; keep a simple note of sleep, mood and side effects; lean on trusted people; and keep daily structure small and kind: daylight, a walk, regular meals. Improvement after a change can take weeks, and partial improvement is still progress worth reporting.

Myth vs fact
If two medicines have not worked, nothing will. — Many people improve on a later step — a different medicine, a combination, added therapy, or treating a condition found alongside. 'Treatment-resistant' means 'needs a fuller plan', not 'hopeless'.

Frequently asked questions
How many treatments must fail before depression is called treatment-resistant?
The usual working definition is not improving enough after at least two adequate antidepressant courses — adequate meaning a proper dose taken for long enough, generally several weeks each.
Should I stop a medicine that is not working?
Do not stop suddenly on your own — stopping some antidepressants abruptly causes withdrawal-like symptoms and relapse. Tell your clinician; changes are planned together, one step at a time.
Is treatment-resistant depression my fault?
No. Brains and bodies respond differently to medicines for many reasons, most outside your control. Your part — honest reporting and keeping appointments — genuinely helps the clinician find what works.
