(Panic Disorders)

Panic disorder means recurring, unexpected panic attacks — sudden surges of intense fear peaking within minutes — plus persistent worry about more attacks or avoiding places where they happened.
Racing or pounding heart, breathlessness, chest tightness, trembling, dizziness, and a feeling of unreality or of losing control during attacks.
A first-ever episode with chest pain or breathlessness should be checked in emergency care the same day. Once panic is diagnosed, treatment works well — seek help rather than shrinking your life around it.

Overview

A panic attack is the body's alarm firing at full volume: adrenaline surges, the heart pounds, breathing quickens, and the mind screams danger — peaking within minutes and passing, usually within half an hour. A single attack can follow stress, illness, caffeine or cannabis, and many people have one or two in a lifetime. Panic disorder begins when attacks recur unexpectedly and the fear of fear takes over: constant scanning of the body, worry about the next attack, and avoiding buses, crowds, markets or being alone. That avoidance — sometimes developing into agoraphobia — is what disables, and it is what treatment targets.

Causes & risk factors

Panic disorder involves a sensitive alarm system (partly genetic), stress that lowers the threshold, and learning: after the first attack, normal body sensations — a faster heartbeat after stairs, warmth in a crowd — get interpreted as the start of another attack, which itself triggers one. Caffeine, energy drinks, cannabis, some medicines and thyroid problems can provoke attacks and are part of the assessment.

How it is diagnosed

A clinician takes a careful history of the attacks and checks the body: heart, thyroid and other causes of similar surges may need examination or tests such as an ECG or blood work. This matters — panic is diagnosed after reasonable physical causes are excluded, which is also why a first episode belongs in same-day care. The pattern of unexpected attacks plus a month of worry or avoidance confirms the disorder.

Treatment options

CBT for panic is highly effective: it teaches what the surge actually is (an adrenaline wave that peaks and passes), retrains breathing, and uses interoceptive exposure — deliberately bringing on harmless sensations like a racing heart through exercise — until they stop being frightening; place avoidance is reversed step by step. Medicines a clinician may use include SSRI/SNRI antidepressants as the daily foundation; sedatives are avoided as a long-term strategy because they teach the brain it cannot cope unaided.

Living with it — self-care

During an attack: remind yourself 'this is adrenaline, it peaks and passes'; slow your out-breath rather than gulping air; loosen clothing; sit if dizzy; let the wave pass like weather — fighting it feeds it. Between attacks: cut caffeine, keep regular sleep, exercise (a fast heartbeat in exercise re-teaches safety), and re-enter avoided places gradually, keeping a record of each success.

When to seek help urgently

Call emergency care or go to hospital the same day for a first episode, any episode with real chest pain, fainting, or severe breathlessness — these must be checked physically first, every time they feel different from usual.

Myth vs fact

A panic attack can make you faint, go mad, or damage your heart. — Panic raises blood pressure and heart rate — fainting is actually uncommon (unlike blood/needle phobia). Attacks feel catastrophic but are the normal alarm system firing wrongly, and they pass. New or different chest symptoms still always deserve same-day medical checks.

Frequently asked questions

Panic attack कति समय टिक्छ?How long does a panic attack last?

Typically it peaks within about ten minutes and fades within twenty to thirty, though the shaky, drained feeling afterwards can last longer. An attack that feels very different, or chest pain that does not settle, should be checked the same day.

Panic disorder खतरनाक छ?Is panic disorder dangerous?

The attacks themselves are not dangerous to a healthy heart, but the disorder shrinks lives through avoidance and raises risks through self-medicating with alcohol. It is very treatable — that is the real message.

मेरो attack का बेला परिवारका सदस्यले के गर्नुपर्छ?What should a family member do during my attack?

Stay calm, speak slowly and simply, remind you this will pass, help you slow your breathing out, and avoid crowding or arguing with the fear. Afterwards, encourage treatment rather than helping you avoid places.
