At a glance — PTSD (Post-Traumatic Stress Disorder)

What it is
PTSD can develop after experiencing or witnessing a traumatic event. The memory fails to file away as past, so the body reacts as if the danger is present: re-experiencing, avoidance, numbness and constant alertness.PTSD
Key symptoms
Intrusive memories, nightmares or flashbacks; avoiding reminders; negative mood and detachment; being easily startled, irritable, and sleeping poorly.
When to seek help
If symptoms persist beyond about a month after the event, or are severe at any point, see a clinician. Trauma-focused therapy is effective — you do not have to live on guard.

Overview

PTSD follows events that involve death, threat of death, serious injury or sexual violence — experienced directly, witnessed, or learned about when they happen to someone close. Nepal's realities give many people exposure: road accidents, earthquakes and floods, conflict in past decades, violence at home, and dangerous migration journeys. In the weeks after such an event, nightmares, jumpiness and intrusive memories are common and usually fade. PTSD is when they persist beyond about a month and keep the person imprisoned by the event: the alarm stays on, reminders are avoided, mood flattens, and life narrows. It is an injury, not a character flaw — and it has specific, effective treatments.

Causes & risk factors

PTSD is caused by the traumatic event itself acting on a vulnerable system — vulnerability shaped by genetics, earlier trauma (especially in childhood), the severity and duration of the event, whether it involved human cruelty, and how much support existed afterwards. It can affect survivors, witnesses, rescue workers, and family members who learn of a violent or accidental death.

How it is diagnosed

A clinician confirms the exposure, the four symptom groups (re-experiencing, avoidance, negative mood/thinking, hyper-alertness), duration beyond a month, and real impact on life — while distinguishing PTSD from depression, anxiety, grief and the normal early stress response (acute stress disorder in the first month). Assessment is paced: you will not be forced to retell everything in detail at the first visit.

Treatment options

Trauma-focused talking therapies are the first-line treatment: trauma-focused CBT and EMDR help the memory finish processing so reminders lose their charge; they are structured and paced, and you stay in control of the speed. Medicines a clinician may use include sertraline, paroxetine or venlafaxine, which can reduce symptom intensity; medicines for nightmares are sometimes considered. Sedatives are generally avoided. Recovery does not require forgetting — it means the memory becomes a memory, not a present danger.

Living with it — self-care

Stability first: regular sleep, meals, gentle movement and limited alcohol (which worsens nightmares and rebound anxiety). Grounding helps during surges — name five things you can see, feel your feet on the floor, remind yourself of today's date and place. Tell one trusted person what helps and what does not (surprises, certain sounds or places). Festivals, anniversaries and news footage can stir symptoms for years; plan for them rather than being ambushed.

Myth vs fact

Myth: PTSD only happens to soldiers.

Fact: Combat is one cause among many. Road accidents, disasters, assault, domestic violence and traumatic childbirth or medical events are common causes in civilian life — and survivors of all of them deserve treatment.

Frequently asked questions

How soon after a trauma do symptoms become PTSD?Trauma

Distress in the first days and weeks is a normal stress reaction. PTSD is diagnosed when the symptom pattern persists beyond about a month. Earlier support still helps — you do not need to wait a month to ask for help if symptoms are severe.

Will trauma therapy make me relive everything?Trauma

Trauma-focused therapy works with the memory in a controlled, paced way — very different from being ambushed by a flashback. You and the therapist control the dose, and stabilisation skills come first for many people.

Can PTSD appear months or years later?PTSD

Yes — delayed onset happens, sometimes triggered by new stress, anniversaries or reminders. Late appearance does not make it less real or less treatable.
